P111A:00 is Mazda’s manufacturer-specific fault code for “Engine Coolant Over Temperature Condition — Forced Limited Power.” When the PCM detects coolant temperature has exceeded its calibrated limit for a set period, it logs the code and drops the engine into a protective limp mode to reduce heat load and prevent damage. It’s not just flagging a warm engine. It’s saying temperatures climbed high enough that the system had to step in and intervene.

The Rising Coolant Level Is the Detail That Matters Most

A coolant level that drops significantly and then appears to rise is not a normal swing. When combustion gas enters the coolant circuit through a failing head gasket, the pressurized exhaust gas forces coolant out of the system and into the overflow tank — making the reservoir level there climb while the main pressure tank or radiator level falls. That back-and-forth is one of the clearest early indicators of a compromised head gasket.

Two checks worth doing on a cold engine before anything else. Remove the reservoir cap, start the engine, and watch for continuous bubbling in the coolant. That’s combustion gas, not trapped air or steam. Then, once the engine is fully warm, watch the exhaust pipe: steady white or grey smoke that persists after the initial cold-start warm-up means coolant is entering the combustion chamber and burning off. Either sign points to a head gasket problem rather than a simple thermostat or sensor fault.

Other Common Causes of P111A on the SkyActiv-G

Thermostat Stuck Closed

If the thermostat fails shut, coolant can’t reach the radiator and engine temperature climbs fast. With the engine fully warmed up, squeeze the upper radiator hose — if it stays cool while the engine is hot, the thermostat isn’t opening. This is the cheapest fix on this list. A thermostat for the SkyActiv-G is an inexpensive part and is always worth ruling out early before going deeper into the diagnosis.

Water Pump Failure

The SkyActiv-G uses a belt-driven water pump. A worn impeller, leaking seal, or seized bearing can all reduce coolant flow enough to trigger overheating. One reliable tell: if the temperature gauge drops when you rev the engine in neutral but climbs again at idle, the pump isn’t moving enough fluid at low RPM. A whining or grinding noise from the front of the engine is another strong indicator.

A Mazda TSB Specific to 2014–2015 Mazda3

Mazda issued a technical service bulletin covering P111A specifically for 2014–2015 Mazda3 vehicles. Some cars left the factory with coolant that wasn’t mixed to the correct concentration. At the wrong ratio, the coolant’s heat capacity drops, and at high engine loads temperature rises faster than expected — triggering P111A with no underlying mechanical failure. Check the concentration with a refractometer or test strip. They’re cheap and give an instant reading. If yours is out of spec, a flush and refill to the correct ratio may be all it needs.

Cooling Fan Not Running

The electric fan should kick on well before temperatures reach the P111A threshold. If it isn’t running at idle or in slow traffic, the engine can’t shed heat even when everything else is working properly. A quick test: switch the AC on. The fan should run immediately. If it doesn’t, start with the relay before replacing the motor.

A Practical Diagnosis Order

  • Cold engine first: check coolant level and inspect the underside of the oil cap for milky or frothy residue — coolant mixing into the oil.
  • Run a chemical block test to check for combustion gas in the coolant. Available at most auto parts stores, takes about fifteen minutes, and gives a clear result.
  • Check coolant concentration with a refractometer or test strip.
  • Warm engine: confirm the cooling fan runs when it should, and feel both radiator hoses — both should be hot once the thermostat opens fully.
  • Upper hose still cool while the engine overheats? Replace the thermostat.
  • Temperature drops when revving in neutral but climbs at idle? Suspect the water pump impeller.

Don’t Keep Driving It

Limp mode exists for a reason. If the head gasket is seeping combustion gas into the coolant, continued driving under load warps the cylinder head — turning a manageable repair into a very expensive one very quickly. The block tester costs next to nothing and takes fifteen minutes. That’s the place to start.
